New patients
This medical questionnaire will help us start to learn about your child or you. Please download this form, answer all questions, even those that require “no” or “none” as answers, and return it to the office by mail or bring it to your appointment. Medical records that you want Dr. Kane to review can also be sent ahead by email or brought with you. We have access to the electronic medical records at Columbia-Presbyterian Medical Center and to some electronic medical records from Weill Cornell Medical Center. We do not use MyChart.
We do our best to minimize wait times for all patients. The nature of this practice sometimes involves urgent surgical care and the consultative responsibilities of an academic medical center. The patience of our parents and patients is supported and appreciated in the event of unexpected schedule changes.
